# How do I check my tax form?

The form opens with a summary of the year: total proceeds, total cost basis, realized gain/loss, and federal income tax withheld, split across six categories of transaction. Your sales appear under the categories that match them, and the rest show dashes. The individual transactions follow, with a sub-total for each digital asset and a total for the form.

Form 1099-DA rows include:

- **Name and code of digital asset:** The name and ticker symbol of the digital asset, followed by its code. Bitcoin’s code is always 4H95J0R2X, a permanent industry identifier for bitcoin itself rather than a number specific to you or your form.
- **Proceeds from QOF:** Marks proceeds from a Qualified Opportunity Fund. Bitcoin sales don’t involve one, so this box will be blank.
- **Number of units:** The quantity of bitcoin sold or disposed of in the transaction.
- **Date acquired:** If bitcoin was transferred from an external source, this may be blank, since the actual acquisition date may be unknown.
- **Date sold or disposed**.
- **Proceeds:** The gross amount you received from selling or disposing of bitcoin.
- **Proceeds is cash only:** Checked when you received only cash, rather than services, other digital assets, or property.
- **Cost or other basis:** The original price you paid for bitcoin. You can [update the cost basis that appears in your form](/support/how-do-i-manage-my-cost-basis) and you remain in control of the cost basis reported on your tax return.
- **Customer-provided information:** Checked when you supplied the acquisition details and Strike relied on them to identify which bitcoin was sold.
- **Number of units transferred in / Transfer in date:** If bitcoin was transferred into your Strike account from another wallet or exchange, these fields show the quantity and date of the transfer.
- **Federal/State tax withheld and State:** Since there is no tax withheld on Strike, these fields will show as dashes.

A single sale can span multiple rows because it can draw on several tax lots. The form’s figures inform [Form 8949](https://www.irs.gov/forms-pubs/about-form-8949) and [Schedule D](https://www.irs.gov/forms-pubs/about-schedule-d-form-1040) of your Form 1040. Each section of the form states whether basis was reported to the IRS, which is what determines the box you select on Form 8949.

To ensure your form is accurate, [update your account details](/support/how-do-i-update-account-details). To correct errors, [contact support](/support/how-do-i-contact-support). If you make a correction before April 15, the new form replaces the original in the app. The IRS advises filing on time even if the corrected form doesn’t arrive on time. If you correct the form after the deadline, an updated copy appears alongside the original and supersedes it in IRS systems.
